SUMMARIZED HOSPITAL COURSE: Patient was admitted to inpatient neurology on 3/09/23 after presenting to the ALM ED with complaints of new onset right sided weakness. BP at presentation was 188/81. No evidence of stroke or other acute abnormalities. CTA did show previously found >80% stenosis of the right internal carotid artery. "He was referred to outpatient vascular surgery to establish care."

severe neuroforaminal narrowing at the L4-S1 levels. Moderate
spinal canal stenosis at L4-L5.

Degenerative disc disease was seen on multiple levels resulting in narrowing at
the neuroforamina (where the nerves exit the back. Moderate spinal canal
stenosis was also seen at the L4-L5 level.
Please let me know if you are interested in being referred to the pain clinic for
consideration of injections or Physical Therapy.

Definition of neuroforamina ---For a lumbar nerve, the neuroforaminal narrowing can affect the nerve roots that make up the sciatic nerve, resulting in sciatica. This narrowing often occurs as part of lumbar spinal stenosis.
Definition of multilevel degenerative disc disease --- Degenerative disc disease is a condition that is categorized by a gradual deterioration and thinning of the shock-absorbing intervertebral discs in your spine. In some cases, disc generation is contained to one overstressed disc, but more often, disc degeneration occurs at multiple levels throughout the spine. When this happens, it’s aptly called “Multi-Level Degenerative Disc Disease.”
Definition of Spinal Stenosis --- Spinal stenosis is the narrowing of one or more spaces within your spine. Less space within your spine reduces the amount of space available for your spinal cord and nerves that branch off your spinal cord. A tightened space can cause the spinal cord or nerves to become irritated, compressed or pinched, which can lead to back pain and sciatica.
